Job summary

Humana's CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home care, evaluate patients, and contribute to treatment planning. The role focuses on rehabilitation in patients' homes, with responsibilities for assessment, documentation, and care coordination.

The position requires a current SLP license, CPR certification, and a valid driver's license with reliable transportation. Home health experience is preferred and the role supports patient independence and quality

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.
• Preferred: Home health experience.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments, including audiologic screening.
• Observe, record and report changes in the patient’s condition and response to treatment to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company.
• Prepare and submit reports and participate in care coordination and discharge planning.
