Job summary

Humana is seeking a Home Health Speech Language Pathologist to deliver in-home therapy, assess and treat communication and swallowing disorders, and contribute to care plans. You will work under the Clinical Manager, coordinating with families and healthcare staff to promote rehab and independence.

The role requires a valid SLP license, CPR, and a driver’s license with reliable transportation. Experience in home health is preferred.

Qualifications

• Six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to minimize effects.
• Administer and interpret diagnostic tests and therapeutic treatments including audio logic screening.
• Observe, record and report changes in patient's condition to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company and/or facility.
• Prepare and submit written reports of evaluations, visits, summaries, care plans and progress reports.
• Participate in care coordination and discharge planning.
