Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in home or facility settings, and to participate in care planning and discharge processes. You will work under the Clinical Manager and ensure documentation and quality of care meet company standards.

The role requires ASHA CCC eligibility or certification, a minimum of six months experience, an active license, CPR, and reliable transportation to patients’ homes.

Qualifications

• Meet ASHA CCC certification requirements for SLP or Audiology.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in home or facility.
• Participate in development and review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapies.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain documentation as required.
• Participate in care coordination and discharge planning.
