Job summary

CenterWell Home Health, part of Humana, seeks a Speech Language Pathologist to deliver in-home care. You will evaluate patients, develop treatment plans, and guide care across home settings to improve communication, swallowing, and quality of life.

You will assess, treat, document progress, and coordinate with supervisors and physicians. A current ASHA CCC or equivalent, licensure, CPR, and valid driver’s license are required for clinic-to-home visits.

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license; CPR certification.

Responsibilities

• Evaluate, direct and provide speech/language pathology services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; document progress and care plans.
