Enhabit Home Health & Hospice is seeking a coordinator to schedule patients with field clinicians and maintain seamless communication among patients, staff, physicians, and referral sources. The role emphasizes care coordination, on-call management, and hospitalization log upkeep to improve continuity of care.

Ideal candidates will have an LPN/LVN license, CPR, and at least one year of healthcare experience, with strong English communication skills and proficiency with common software and mobile

Qualificações

• High School Diploma or equivalent.
• Licensed practical or vocational nurse.
• Valid clinical license per state rules.
• Minimum of one year of clinical experience in healthcare.
• CPR certification.
• At least one year of home health, hospice, or pediatric experience within the last 24 months with staffing and scheduling experience.

Responsabilidades

• Schedule patients to field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain care coordination and continuity of care.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
