Enhabit Home Health & Hospice is seeking a Scheduling Coordinator to align patients with field clinicians and ensure seamless care coordination. You will work closely with field staff, patients, physicians, referral sources, and caregivers to maintain continuity of care and effective communication.

The role requires a background as an LPN or similar, a valid clinical license, CPR, and a track record in home health or hospice environments.

Qualifications

• High School Diploma or equivalent is required.
• Licensed Practical Nurse (LPN) license is required.
• Must have and maintain a valid clinical license per state rules.
• Minimum of one year of clinical healthcare experience.
• CPR certification is required.
• Minimum of one year of home health, hospice, or pediatric experience within the last 24 months with staffing/scheduling experience.
• Valid state driver license is required.
• Automobile liability insurance is required.
• Basic demonstrated technology skills including mobile device operation.

Responsabilités

• Scheduling patients to field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain care coordination and continuity.
• Manage the on-call notebook and hospitalization logs to enhance stakeholder communication.
