Job summary

Best Home Health Providers, Inc. is seeking a per diem Occupational Therapist to support patients in the Bay Area and beyond. You will evaluate patients in their homes, develop plans, and guide daily living activities to restore independence and function.

Responsibilities include teaching patients, coordinating with physicians, and supervising support staff. A CA OT license and at least 2 years of OT experience are required; travel is part of the role.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA/ AOTA accreditation bodies.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient's treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
