Job summary

Best Home Health Providers Inc. is seeking an Occupational Therapist to support patients in their homes across the Bay Area. The role focuses on helping patients regain independence and safely navigate daily activities through personalized therapies.

Responsibilities include evaluation, treatment planning, family education, and coordination with the care team. A CA OT license, CPR certification, and 2+ years of experience are required, with per diem visits at $110–$120 per visit.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the AMA/AOTA
• Registered with the National Registration Examination of AOTA
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise OTAs and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services
