Best Home Health Providers, Inc. is seeking an Occupational Therapist to support patients at home across the Bay Area and beyond. This per diem role focuses on helping patients regain independence and safety in daily living activities.

You will evaluate homes, develop personalized treatment plans, teach skills, and collaborate with physicians and care teams to ensure seamless patient-centered care. A California OT license and at least 2 years' experience are required.

Qualifications

• Graduate of an occupational therapy curriculum accredited by national and state bodies.
• Active California OT license and current CPR certification with AHA.
• Minimum 2 years of clinical OT experience.

Responsabilités

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment and orthotic devices.
• Maintain accurate clinical records and report changes in patient condition to the team.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient's treatment approach.
• Supervise OT assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
