Job summary

Best Home Health Providers Inc. seeks an Occupational Therapist to support patients at home in the Bay Area and beyond. You will help patients regain independence, improve function, and safely navigate daily life, all within a family-oriented environment.

The role is per diem with pay per visit, and opportunities exist to convert to full-time based on performance. Prior OT experience, California license, and CPR certification are required.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the relevant national and professional bodies.
• Registered with NBCOT and licensed to practice in California.
• Active California OT license and CPR certification (AHA).
• Minimum 2 years of clinical OT experience.
• Valid driver’s license and reliable insured vehicle with ability to travel to patient homes.
• Excellent communication and organizational skills.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with the care team for seamless, patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the skilled service.
• Support discharge planning as patients transition from home health care.
• Perform other duties as required for patient services.
