Job summary

Best Home Health Providers Inc. is seeking a per diem Occupational Therapist to support home-based patient care across the Bay Area and beyond.

The role emphasizes evaluating home environments, creating personalized plans, and teaching patients daily living skills in their own homes. The position requires CA OT licensure, CPR, and at least 2 years of OT experience with strong communication and organizational abilities.

Qualifications

• Must hold an Occupational Therapy degree and a valid California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in activities of daily living
• Recommend adaptive equipment and devices
• Maintain accurate clinical records and report changes in patient condition
• Coordinate with the care team for patient-centered care
• Educate family members and caregivers on treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is sole skilled service
• Support discharge planning as patients transition home
• Perform other duties as required for patient services
