Best Home Health Providers, Inc. is seeking an Occupational Therapist to provide in-home care across the Bay Area and beyond.

This per diem role focuses on helping patients regain independence, improving function, and safely navigating daily life in their own homes. Responsibilities include evaluating home environments, creating personalized treatment plans, teaching daily living skills, and coordinating with the broader care team for seamless patient-centered care.

Qualifikationen

• Graduate of an occupational therapy curriculum accredited by the relevant US bodies.
• Registered with the national occupational therapy registration exam.
• Active California Occupational Therapy license required.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license with reliable insured vehicle for home visits.

Aufgaben

• Evaluate patients in their home to identify safety needs and functional limitations.
• Develop personalized treatment plans and communicate with physicians and care team.
• Teach and retrain patients in daily living activities affected by illness or injury.
• Recommend adaptive equipment and orthotic devices as needed.
• Maintain accurate clinical records and report changes promptly.
• Coordinate with the care team to ensure seamless patient-centered care.
