Job summary

Best Home Health Providers, Inc. seeks an Occupational Therapist to support patients recovering at home across the Bay Area.

The role focuses on evaluating functional abilities, creating personalized plans, and guiding patients in daily living activities to restore independence in comfortable home settings. The successful candidate will have an OT degree, CA licensure, active CPR, and at least two years of clinical OT experience, with reliable transportation to patient homes.

Qualifications

• Graduate of an OT curriculum accredited by AMA/AOTA.
• Licensed in California for Occupational Therapy.
• Minimum 2 years of clinical OT experience.
• Current CPR certification (AHA).
• Valid driver's license and reliable vehicle for home visits.

Responsibilities

• Evaluate patients' functional abilities and home environments for safety needs.
• Develop and communicate personalized treatment plans to physicians and care team.
• Teach and retrain patients in activities of daily living affected by illness or injury.
• Recommend adaptive equipment and orthotic devices; provide instruction on use.
• Maintain accurate clinical records and report changes in patient condition promptly.
• Coordinate with the broader care team for seamless, patient-centered care.
• Educate family and caregivers on treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the skilled service.
• Support discharge planning as patients transition home.
• Perform other duties as needed for patient services.
