Best Home Health Providers Inc. is seeking a per diem Occupational Therapist to support patients in home settings across the Bay Area and surrounding regions. You will evaluate functional abilities, design therapy plans, and guide patients in daily living activities to promote independence and safe home navigation.

Qualifikationen

• Graduate of an OT curriculum accredited by the appropriate American bodies.
• Registered to take or passed the National Registration Examination for OT.
• Active California OT license required.
• Minimum 2 years of clinical OT experience.
• Current CPR certification per AHA guidelines.

Aufgaben

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the only skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
