Job summary

Best Home Health Providers Inc. is seeking an Occupational Therapist to deliver high‑quality home health care across the Bay Area. The role is per diem, visiting patients in their homes to improve independence and daily functioning.

You will develop personalized treatment plans, educate families, supervise assistants, and contribute to smooth discharge planning. California OT license and NBCOT certification are required, with travel to patient homes.

Qualifications

• Graduate of ACOTE‑accredited OT curriculum.
• Registered with the NBCOT (National Registration Examination).
• Active California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients' functional abilities and home environments for safety.
• Develop and communicate individualized OT treatment plans.
• Teach and retrain ADLs (daily living skills).
• Instruct on adaptive equipment and splints.
• Maintain accurate clinical records and report changes.
• Coordinate with care team for patient-centered care.
• Educate family and caregivers on treatment approach.
• Supervise OT Assistants and oversee home health aides when OT is the sole skilled service.
• Support discharge planning as patients transition home.
• Perform other duties as required.
