Job summary

Enhabit Home Health & Hospice is seeking a dedicated Referral Coordinator to support patient referrals and serve as a resource for referral sources. You will collaborate with branch and sales teams to ensure smooth transitions to services and apply regulatory guidelines.

Ideal candidates have a high school diploma (RN preferred), at least two years in healthcare with home health or pediatrics exposure, strong customer service, and basic tech skills to operate mobile devices and electronic

Qualifications

• Must have demonstrated experience in customer service.
• Must have knowledge of Medicare guidelines that govern home health or hospice.
• Must have basic technology skills, including operation of a mobile device.
• Education and experience preferred: licensed nurse and graduation from an approved nursing program.
