Enhabit Home Health & Hospice is seeking a Referral Coordination professional to manage patient referrals and act as a liaison for referral sources. The role supports branch and sales teams to ensure seamless coordination and transition to services, while applying knowledge of Medicare guidelines for home health/hospice.

The ideal candidate will have a high school diploma, healthcare experience including home health or pediatrics, strong customer service skills, and basic tech proficiency,

Qualifications

• Must possess a high school diploma or equivalent.
• Two+ years in health care with at least one year in home health or pediatrics.
• Demonstrated experience in customer service.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic technology skills and comfort with mobile devices.
