Job summary

Enhabit Home Health & Hospice is seeking a dedicated scheduling coordinator to arrange patient visits for field clinicians and coordinate care across patients, staff, physicians and referral sources. This role emphasizes clear communication, on-call management, and accurate hospitalization logs to support seamless care delivery.

The ideal candidate has a high school diploma, an active nursing license (LPN/LVN), CPR certification, and at least one year of healthcare experience; in-office

Qualifications

• High School Diploma or equivalent.
• Licensed practical nurse (LPN) with valid clinical license.
• CPR certification.
• Minimum one year of clinical experience in healthcare.
• Valid state driver license and automobile liability insurance.
• Basic technology skills including mobile device operation.
• Experience with Homecare Homebase (preferred).
• Bilingual English/Spanish preferred.

Responsibilities

• Schedule patients to field clinicians and coordinate care across patients, staff, physicians, referral sources and caregivers.
• Maintain on-call notebook and hospitalization logs to support communication among stakeholders.
• Ensure care coordination and continuity of care across multiple teams.
