Job summary

Enhabit Home Health & Hospice is seeking a scheduling professional to align patients with field clinicians and maintain strong care coordination. You will communicate with patients, staff, physicians, referral sources and caregivers to ensure seamless service delivery and continuity of care.

The role requires a valid LPN/LVN license (or equivalent), CPR, and 1+ year of clinical/home health experience, with in-office attendance essential.

Qualifications

• High School Diploma or equivalent required.
• Licensed Practical Nurse (LPN/LVN) license active per state rules.
• CPR certification required for the role.
• 1+ year clinical healthcare experience.
• 1+ year home health, hospice, or pediatric experience in last 24 months.
• Valid driver license and auto liability insurance.

Responsibilities

• Schedule patients to field clinicians to ensure timely care coordination.
• Communicate with patients, field staff, physicians, referral sources and caregivers.
• Manage on-call notebook and hospitalization logs to support coordination.
