Job summary

ForzaCare in Nashville is seeking a Field Medical Case Manager to coordinate care for injured workers, connecting them with medical providers, employers, and insurers to ensure timely, transparent care and safe return to work.

Ideal candidates are licensed RNs or CRCs with workers' compensation experience; CCM or CIRS credentials are a plus. Travel to appointments is required, and you will document goals and progress for regulatory compliance.

Qualifications

• Active RN or Certified Rehabilitation Counselor license with degree is required.
• CCM, CIRS or other case management credentials preferred.
• Must comply with state licensure and certification requirements.
• Prior workers' compensation case management experience strongly preferred.
• Valid driver's license and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including visits to workplaces.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and team members.
