Job summary

Housing Works in New York is seeking a Housing Case Manager for the Emergency & Transitional Housing Program (ETH). You will conduct psychosocial assessments, co-develop housing and treatment plans, and assist participants with housing placement and benefits enrollment.

You will manage a caseload of 35–45 clients, coordinate with health and social services, ensure up-to-date charts in our EMR, and participate in program meetings and quality assurance activities.

Qualifications

• Bachelor’s Degree in social services, psychology, social work or related field.
• Experience providing case management or direct client service.
• Experience with clients involved in the criminal legal system and with substance use or mental health diagnoses.
• Strong written documentation and communication skills.
• Microsoft Office Suite, particularly Excel, required.
• Experience utilizing EMR systems.

Responsibilities

• Manage a caseload of approximately 35-45 clients enrolled in the ETH Program.
• Conduct psychosocial assessments with all newly enrolled clients.
• Develop housing plans of care and conduct 30-day plan reviews.
• Assist clients with obtaining government IDs, SSNs, birth certificates, etc.
• Assist clients with benefits enrollment and housing applications.
• Coordinate with internal and external health, wellness or other support services.
• Maintain up-to-date client charts in Housing Works EMR.
• Submit reports and participate in meetings and quality assurance activities.
