Job summary

Eden Housing in Castro Valley, CA is seeking an experienced Case Manager to oversee resident services across properties serving high-need individuals, families, seniors and special needs populations. You will coordinate services and build partnerships with county providers to bring robust programming to our communities.

Under supervision of the Associate Director of Supportive Housing, you will deliver onsite programs, conduct outreach, support crisis intervention, and promote independent living

Qualifications

• BA/BS in social work, psychology, counseling, or related field required.
• Master’s degree in same fields preferred.
• At least 3 years' experience serving senior populations and those with special needs.

Responsibilities

• Deliver onsite programs meeting internal standards and regulatory expectations.
• Regularly conduct outreach by door-to-door, phone, and email to raise awareness of services offered.
• Assist in crisis intervention and follow-up, including mandated reporting policies and procedures.
• Support case management services for residents with functional impairment and help develop independent living skills.
• Provide group and one-on-one services and support community-building activities at properties.
• Collaborate with residents and ILS/SLS workers to meet residents' needs and goals.
