Job summary

PATH is seeking a Case Manager in Winnetka, California to join the Supportive Housing Services team. The role focuses on intensive case management, assisting clients through homelessness to permanent housing with an emphasis on housing goals and coordinated services.

Responsibilities include intake, ISP development, referrals, and maintaining confidential documentation. Driving is required; full-time, non-exempt status applies, with a comprehensive benefits package.

Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.

• Completes program specific assessments (e.g. VI-SPDAT).
• Evaluates strengths and challenges; conducts 90‑day reviews with clients.
• Maintains documentation standards and keeps complete client files per PATH policies.
• Maintains client confidentiality per HIPAA and applicable guidelines.
