Job summary

ACMH seeks a Senior Case Manager for the Bronx to coordinate case management and deliver community rehabilitation and housing support services. The role reports to the Program Director and requires home visits, client assessments, and collaboration with psychiatric care providers.

Ideal candidates hold an MA or BA with relevant experience in mental health, and demonstrate strong written and verbal communication skills.

Qualifications

• MA in psychology or a human service related field, or BA with at least 3 years of direct care experience.
• Experience working with adults with serious mental illness preferred.
• Strong written and verbal communication skills.
• Master's degree preferred.

Responsibilities

• Conducts home visits for assigned caseload.
• Obtains psychosocial histories and conducts functional skills assessments.
• Liaises with primary psychiatric care providers and ensures access to psychiatric, medical and entitlement services.
• Monitors client medication adherence and reports side-effects.
• Delivers community rehabilitation and housing support services.
• Documents client progress and updates the Service/Support Plan.
• Trains clients in safety procedures and prepares residents for independent living.
• Provides crisis intervention and coverage for other staff as needed.
