ACMH INC in The Bronx is seeking a Senior Case Manager/Case Manager to coordinate care and provide rehabilitation services. The role reports to the Program Director and involves home visits, housing support, and crisis intervention within a community-based program.

The position requires graduate-level education in psychology or related field, relevant experience with adults with serious mental illness, and strong communication skills. Generous benefits are offered.

Qualifications

• M.A. in psychology or a human service related field.
• B.A. in psychology or a human service related field with a minimum of 3 years of direct care experience.
• Experience working with adults with serious mental illness preferred.
• Strong written and verbal communication skills.
• Master’s Degree preferred.

Responsabilités

• Regular home visits for assigned caseload.
• Obtain psychosocial histories and conduct functional skills assessments.
• Liaise with psychiatric care providers.
• Ensure access to psychiatric, medical, and entitlement services.
• Monitor medication adherence and side-effects.
• Develop individualized rehabilitation plans.
• Deliver housing and community rehabilitation services as needed.
• Document responses and update the Service/Support Plan.
• Train clients in safety procedures and crisis intervention.
