Job summary

The Salvation Army in Indiana is seeking a Temporary Case Manager to support families and individuals in Lake County with emergency financial services, pantry aid, and case management under the Pathway of Hope model.

You will maintain documentation in SIMS and other systems, coordinate referrals, and collaborate with local social service providers to fulfill the mission with a faith-based approach.

Qualifications

• Associate degree in Human or Social Services or related field required.
• Four years of social services work experience can substitute for the degree.
• Background checks required and reviewed.
• Valid driver’s license and specified trainings must be completed within set timelines.
• Strong communication, computer skills, and ability to serve diverse populations.

Responsibilities

• Schedule and conduct intake appointments for case management services.
• Implement strengths-based, client-centered case management to identify goals and barriers.
• Refer households to Pathway of Hope program to support long-term goals.
• Maintain referral sources across the county to provide resources.
• Complete and maintain documentation in SIMS and other systems per policies.
• Network with other social service providers and represent the Salvation Army at functions.
