Job summary

Qualifications

• Bachelor's degree or equivalent combination of training and experience.
• 2+ years of applicant interview or case management experience.
• Experience with program target population preferred.
• Adhere to ethics codes and maintain professional boundaries.
• Maintain client confidentiality and release of information.
• Complete Safe From Harm training and stay current.

Responsibilities

• Facilitates case management within program guidelines and develops case plans.
• Supports participants, tracks progress, and coordinates referrals.
• Maintains relationships with community agencies and promotes services.
• Creates case plans and helps participants achieve identified goals.
• Maintains accurate case records and manages service transactions.
• Responds to surveys and provides information for admin reports.
• Performs other duties as assigned.
