Job summary

VOALA is seeking a Case Manager Generalist to provide on-site case management for low acuity clients, coordinating services and developing individualized plans to address housing, health, employment, and essential supports.

The role requires strong screening, advocacy, and collaboration with service teams, with a commitment to serving vulnerable populations and ensuring access to resources through VOALA's programs.

Qualifications

• Requires fingerprint clearance and background check.
• Ability to work with difficult, vulnerable clients with empathy and professionalism.
• Familiarity with case management processes and social services coordination.

Responsibilities

• Conduct comprehensive assessments to collect client data and needs.
• Coordinate care plans and connect clients to housing, health, employment, and other services.
• Advocate for clients with systems and providers; participate in case conferences.
• Ensure on-site service delivery and collaboration with the service team.
