Job summary

VOALA is seeking a Case Manager (Specialist) to provide intensive case management to eligible clients, meeting weekly and coordinating services on-site in Los Angeles area. You will handle a caseload and connect clients with housing, health, education and other resources while advocating for their needs.

Requirements include an Associate's Degree in Social Work or related field and at least 1 year of case management experience, with VOALA training completed within six months.

Qualifications

• Associate's Degree in Social Work or related field or combination of education and relevant experience.
• Completion of VOALA Case Management training within 6 months of hire.
• Minimum 1 year of general experience in case management or similar social services.
• Ability to pass fingerprint clearance and background checks.

Responsibilities

• Conduct screenings and assessments to collect information for case planning.
• Provide service linkages and support to meet identified needs.
• Coordinate planning with clients to meet short- and long-term goals.
• Coordinate case conferences, monitor progress, and advocate for clients where needed.
• Document group education sessions clearly and concisely.
