VOALA is seeking a Case Manager (Specialist) to provide intensive case management to eligible clients, meeting participants at least weekly and coordinating services across housing, employment, and health resources. The role requires on-site presence and fingerprint clearance as part of safeguarding vulnerable populations.

You will develop action plans, document progress, advocate for clients, and collaborate with service teams to connect individuals to essential resources and supports.

Qualifications

• Must be able to pass a fingerprint clearance and background check.
• Ability to work with challenging clients and provide compassionate support.
• On-site work requirement and community-based service delivery.

Responsabilités

• Conducts comprehensive screenings and assessments to develop individualized case plans.
• Provide service linkages and support systems to meet identified needs.
• Coordinate case conferences and advocacy across agencies.

Formation

Associate’s Degree in Social Work or related field

Two years’ total education/experience
