Job summary

The People Concern in Los Angeles is seeking a Case Manager for the ODR Project Based Housing to support clients transitioning from homelessness to permanent supportive housing. You will provide intensive case management and coordinate resources to promote independence and housing stability.

This full-time role requires strong documentation, HIPAA compliance, and collaboration with DHS/ODR teams. A compassionate, trauma-informed approach is essential for success in a busy urban setting.

Qualifications

• High School Diploma or GED or equivalent.
• Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record.
• Able to pass a background check for jail clearance.
• Able to transport clients in your vehicle.
• Bachelor’s degree preferred; or a minimum two (2) years’ case management experience working with individuals experiencing homelessness.
• Demonstrated knowledge of case management.
• Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers.
• Skilled in non-violent crisis intervention.
• Computer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs.
• Detail oriented with strong time management, organizational, written, verbal, interpersonal, and computer skills.
• Able to obtain and maintain CPR/First Aid certification.
• Minimum 30 WPM typing speed preferred.

Responsibilities

• Work with a diverse and marginalized client caseload of 15 justice-involved clients experiencing many barriers.
• Maintain weekly contact with all clients and document contact in County approved database.
• Ensure that case management services reflect Harm Reduction and trauma-informed care.
• Link clients to medical, mental health, substance abuse, and vocational resources.
• Assist clients in obtaining all benefits to which they are entitled.
• Participate in court hearings and submit court progress reports.
• Ensure client biopsychosocial assessments are up-to-date.
• Develop goal plans with clients and review/adjust quarterly.
• Maintain client files with required documentation in records and databases.
• Document all client contacts in PIP format per productivity expectations.
• Maintain confidentiality of client files per HIPAA guidelines.
• Coordinate appointments, transportation, and follow-up services for health care and community resources.
• Transport clients to appointments with providers in the LA area.
• Develop trusting relationships to support independence and health maintenance.
• Maintain knowledge of community resources for wrap-around services.
• Assist clients with daily activities, scheduling, budgeting, and recovery progress.
• Work with staff teams and attend weekly meetings and trainings.
