Job summary

Qualifications

• Bachelor’s degree in Human Services or related field or equivalent experience.
• Certification in Case Management or related field preferred.
• Strong communication, organization, and confidentiality in handling sensitive data.

Responsibilities

• Advocate for clients and deliver services with safety, respect, and effectiveness.
• Access community resources, schedule and attend appointments; maintain documentation.
• Develop client-centered goals and case plans with measurable objectives.
• Coordinate with shelter programs, outreach teams, and partner agencies to promote housing stability.
• Maintain confidentiality and manage 20–30 active cases with regular progress reviews.
