Position Summary

The Case Manager serves as the primary client-centered service professional responsible for guiding families from homelessness to housing stability and long-term independence. This position provides trauma-informed, goal-driven case management services that address housing, employment, education, financial stability, childcare, transportation, and behavioral health barriers.

The Case Manager develops individualized service plans, monitors measurable progress, and ensures compliance with HUD, Continuum of Care (CoC), and Providence House standards. Through structured accountability, coordinated referrals, and consistent follow-up, this role directly supports sustainable housing outcomes and family self-sufficiency.

Key Responsibilities
Client Case Management
  • Conduct comprehensive intake assessments to identify strengths, barriers, and service needs.
  • Develop individualized case plans with measurable goals related to housing stability, employment, education, and family well-being.
  • Meet with clients regularly (minimum bi-weekly or as needed) to monitor progress and adjust goals.
  • Provide trauma-informed support, motivational coaching, and accountability guidance.
  • Maintain manageable caseloads assigned by the Director of Housing.
  • Document all services, case notes, and progress updates in HMIS and internal systems within required timelines.
  • Coordinate referrals to community partners including healthcare, financial counseling, legal aid, workforce development, and childcare resources.
  • Facilitate life skills, financial literacy, and housing readiness workshops as assigned.
Housing Stability & Transition Support
  • Collaborate with the Housing Team to identify appropriate housing placements.
  • Assist clients with housing applications, landlord communication, lease preparation, and move-in planning.
  • Conduct home visits post-placement to monitor stability and identify emerging needs.
  • Maintain structured follow-up for 90 days post-exit to support long-term housing retention.
  • Travel throughout the community to conduct home visits, attend partner meetings, accompany clients when appropriate, and provide supportive services related to housing stability and self-sufficiency.
Data Management & Compliance
  • Enter and update client information in HMIS with accuracy and confidentiality.
  • Ensure all documentation complies with HUD, CoC, and Providence House requirements.
  • Participate in internal audits, data quality reviews, and compliance monitoring.
  • Submit client outcome data to support grant reporting and performance measurement.
Collaboration & Professional Accountability
  • Participate in case conferencing, multidisciplinary team meetings, and staff development sessions.
  • Communicate effectively with Directors and cross-department staff to provide coordinated wraparound services.
  • Advocate for clients while maintaining professional boundaries and ethical standards.
  • Support internal and external audits with accurate and timely documentation.
  • Maintain strict confidentiality in accordance with HIPAA and agency policies.
Education and Experience

Bachelor’s degree in social work, Human Services, Psychology, or related field preferred.

Minimum of two (2) years of experience in case management, housing services, or related human

Familiarity with HMIS and HUD/CoC program requirements strongly preferred.

Experience working with families experiencing homelessness or poverty-related barriers preferred.

Valid driver's license required. Candidates without a current valid driver's license must demonstrate an active plan to obtain or reinstate driving privileges within a timeframe approved by Providence House.

Must maintain driving privileges and remain insurable under Providence House's automobile insurance requirements throughout employment.
