Job summary

RHA Health Services, LLC seeks a Blended Case Manager to support children and adults with intellectual and developmental disabilities, mental health, and substance use challenges. You will coordinate services, advocate for clients, and collaborate with community providers in Bethlehem, NY.

The role includes 24/7 on‑call coverage, charting, and securing entitlements while ensuring timely documentation and service plan development.

Qualifications

• Bachelor’s degree in a human services field or an Associate’s with 12 credits and two years in mental health/human services.
• Valid driver’s license is required.

Responsibilities

• Advocate for clients and help negotiate complex systems such as special education and social services.
• Assist clients in maintaining and accessing basic living needs and skills including employment, housing, food, medical care, and recreation.
• Provide crisis intervention and 24/7 on‑call case management support.
• Develop and revise client service plans, treatment plans, and individual crisis plans.
• Document progress, complete assessments, new admissions, medication records, incident reports, and other required paperwork.
• Assist clients in securing funding and entitlements for needed services.
• Collaborate with county Office of Mental Health/IPAS personnel and other service providers to assess client needs.
• Maintain accurate case records and meet charting timeframes.
