Qualifications

• Bachelor's degree in human services or related social sciences.
• Must maintain a valid PA Driver's license.

Case management work is done in communities settings, not in offices.

The Blended Case Manager works with the serious and persistently mentally ill population, using a strength-based perspective, to assist clients to obtain necessary services and support. The primary goal is to empower the client to identify their needs and choose the resources to fulfill them.

Job Type: Full-time, flexible hours

Pay: Starting at $33,000 per year; up to $37,000 with experience

Requirements

Bachelor's degree with major course work in human services or other related social sciences. Must maintain a valid PA Driver's license.

A Blended Case Manager should possess the following skills
