CENTER FOR COMMUNITY RESOURCES INCORPORATED in Sunbury, PA seeks a Blended Case Manager to provide services to individuals with mental health needs. The role includes coordinating access to services, documentation, and ensuring quality standards while supporting clients at home and in the community.

The position requires a 4-year degree in Human Services or related field and at least one year of MH case management experience, with ability to travel and participate in on-call coverage.

Qualifications

• Bachelor’s degree in Human Services or related field.
• One year experience in MH Case Management or mental health field.
• Knowledge of case management principles, Recovery Model, and local resources.
• Valid driver’s license and auto insurance.
• Act 33/34 and FBI clearances required.

Responsibilities

• Provide blended case management services to individuals with mental health diagnoses.
• Complete reports and documentation in a timely fashion.
• Maintain quality standards.
• Participate in on-call rotation for 24-hour coverage.
• Travel in the community; work in office, community, remote settings if applicable.
