Job summary

RHA Health Services, LLC is seeking a full-time Blended Case Manager to assist children and adults facing intellectual, developmental disabilities, and mental health challenges. In this role, you will help connect clients with local resources and provide crisis intervention services.

Candidates should have a relevant Bachelor’s degree or an Associates degree with applicable experience. Strong communication skills and the ability to work with diverse populations are essential.

This position offers competitive pay at $20.00/hr., a sign-on bonus, and a generous benefits package including paid time off and medical coverage.

Qualifications

• Bachelor’s degree in sociology, social work, psychology, or related field OR Associates degree with relevant experience.
• Valid Pennsylvania driver’s license with clean record.
• Ability to pass background checks.

Responsibilities

• Assist consumers in gaining access to resources/services.
• Communicate weekly to determine progress and revise service plan.
• Develop and enhance consumer social support networks.
• Provide 24/7 on-call crisis services.
