Job summary

Elwyn is seeking Blended Case Managers to provide outreach to individuals in their home setting who have serious and persistent mental illness. A key component is to work with individuals to access, link, coordinate and monitor needed services, while offering support and advocacy to promote independence and stability.

Documentation is important and must reflect person-centered goals based on the service/treatment plan, with timely records and collaboration within a multi-disciplinary team.

Qualifications

• Bachelor's Degree in a related field is required or preferred.
• Experience in community-based mental health services is valued.
• Experience with case management is encouraged.
• Computer literacy is required.
• Knowledge of crisis intervention principles.
• Knowledge of cognitive and behavioral aspects of mental illness.
• Must have a valid driver's license and driving record.

Responsibilities

• Preparing and maintaining timely, accurate, and complete records.
• Participating as a team member to improve overall effectiveness.
• Ensures interventions and supports are delivered with consistency, flexibility, and responsiveness to changing client needs.
• Involves clients in identifying and coordinating treatment issues.
• Empowers clients through therapeutic rapport, advocacy, and skill building.
• Exhibits proficiency with benefit and entitlement options to maximize treatment planning opportunities.
• Coordinates emergency psychiatric services within a timely manner, including documenting interventions and response.
