Job summary

Horizon House, Inc. is seeking a Blended Case Manager in Delaware County to ensure people receive essential services in their local community. You will develop rehabilitation plans covering social, educational, vocational and residential skills and monitor progress.

The role involves crisis intervention, advocacy, and coordinating with providers, with travel as needed. A BA/BS in a related field plus MH experience is required, and strong communication and computer skills are essential.

Qualifications

B.A/B.S. degree in Human Services/Social Science, Healthcare or related field and 3 years MH direct care experience; or combination of experience/education.
Excellent communication skills and accuracy in documentation.
Ability to work independently and within a focused team.
Proficiency with computers, email and basic EHR systems a plus.

Responsibilities

Ensure individuals receive needed services to live, learn, work and socialize locally.
Develop and refine individual rehabilitation plans including social, educational, vocational and self-care goals.
Complete Functional Assessments and review service accessibility.
Assist in crisis resolution and coordinate with key supports or direct intervention.
Advocate for clients and maintain liaison with community providers and agencies.
