Qualifications

• Bachelor’s degree in Human Services or related field.
• One year of experience in mental health case management.
• Knowledge of case management principles, computer literacy, familiarity with the Recovery Model, and knowledge of local resources.
• Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances.
• Act 33/34 and FBI clearances.

Responsibilities

• Provides case management to adolescents, children, and adults with serious mental illness or emotional disturbance.
• Links, coordinates, and monitors needed services; advocates for client independence.
• Completes reports and documentation timely and maintains quality standards.
• Participates in on‑call rotation for 24‑hour coverage.
• Manages time to meet consumer and program needs; handles monthly utilization and documentation.
