Job summary

Urban Pathways, Inc. is seeking a part-time Case Aide in Queens, NY. The role supports the social services team by handling client intakes, referrals, and documentation while promoting an inclusive, anti-racist environment.

You will assist with housing and medical referrals, maintain client charts, and accompany clients to appointments. A HS diploma or GED is required, with preferred experience in homeless services.

Qualifications

• High school diploma or GED required; associate degree preferred.
• Experience in social services or with at‑risk populations preferred.
• Strong written and verbal communication skills.
• Computer skills including MS Word/Outlook.

Responsibilities

• Conducts client intakes and updates information in client database system(s).
• Screens clients for referral and case management services.
• Screens client arrivals to identify immediate needs, provides services and/or makes appropriate referrals as necessary.
• Organizes, maintains and prepares client charts for intake and discharge.
• Handles caseload in the absence of the Case Manager as mandated by program.
• Conducts homeless outreach and assesses client needs.
• Documents client activities and updates client information in database system(s).
• Escorts clients to appointments, including medical, housing, and entitlement appointments.
