Job summary

Urban Pathways in New York, NY seeks a Case Manager to provide and document clinical and case management services for assigned clients. The role emphasizes active listening, high-quality customer service, and cross-cultural awareness to promote an equitable, anti-racist organizational culture.

Responsibilities include conducting intake, developing service plans, making home visits, coordinating referrals, monitoring medications, and maintaining timely documentation in client records.

Qualifications

• Bachelor’s degree required. Bachelor’s degree in Social Work or other related field preferred.
• Minimum one year of case management, individual or group counseling experience preferred.
• Knowledge of NYC housing subsidy programs.

Responsibilities

• Provides comprehensive case management services for assigned clients including, but not limited to: support and resources/referrals for clients to meet objectives; Employment/Education/Vocational Training; Health, mental health treatment, and substance abuse services; Social and community integration.
• Counseling and support to clients on an on-going basis including, but not limited to: Crisis intervention; Training and support on client daily living skills; Assistance in problem solving; Conflict resolution.
• Assesses client needs, develops service plans and/or service referrals to achieve client self-sufficiency; Conducts field work including home visits and outreach; Monitors client medications as required; Conducts client intake and updates information in client database systems; Completes incident reports and distributes related notices within 24 hours.
