Job summary

Catholic Charities is seeking a Case Manager to support individuals in navigating services and resources, promoting wellness and stability. The role involves documenting case records, intake screening, housing assessments, ISP development, and progress tracking.

Part-time position with a 25-hour work week in the New York area. Qualifications include an Associate's Degree in Social Work (Bachelor's preferred) and 1–3 years in Human Services; Spanish bilingualism is preferred.

Qualifications

• Education: Associate's Degree in Social Work; Bachelor's degree preferred.
• Experience: 1–3 years in Human Services or related field; bilingual Spanish preferred.
• Valid Driver's License with car insurance and access to a vehicle.

Responsibilities

• Complete timely and accurate documentation in the Uniform Case Record and related intake materials.
• Review intake information for new referrals, perform initial screening, and make recommendations on acceptance/denial.
• Conduct Housing Office intake assessments for new tenants and annually thereafter.
• Develop and implement the ISP and report outcomes every 6 months with changes as needed.
• Address monthly progress against milestones related to housing and services.
• No subject-to-call shifts.
