Job summary

CVS Health is seeking a Case Manager in Illinois to join our team and coordinate comprehensive member care. You will assess needs, develop and monitor care plans, and collaborate with healthcare providers and community resources to improve outcomes for members with complex health and social needs.

The role requires a Master’s in Behavioral/Mental Health or related field, 3–5 years of clinical experience, and the ability to work remotely with substantial travel within designated areas.

Qualifications

• Master's degree in Behavioral/Mental Health or related field.
• 3-5 years clinical practical experience.
• 2-3 years CM, discharge planning and/or home health care coordination experience.
• Confidence working at home/independent thinker, using tools to collaborate remotely.
• Must reside in Illinois and willing to travel up to 50-75% in designated areas.
• Proficiency with MS Word, Excel, Outlook and PowerPoint; ability to navigate multiple systems.

Responsibilities

• Assess member needs and eligibility using clinical tools and data.
• Coordinate care plans and facilitate collaboration among providers.
• Apply clinical guidelines to ensure appropriate benefits and care decisions.
• Provide crisis intervention and follow-up as needed.
• Document care plans and monitor progress with care teams.
