CVS Health in Illinois is seeking a full-time Case Manager to assess members' needs, coordinate benefits, and advocate for patients facing complex health and social challenges. You will collaborate with clinicians and community partners to address comprehensive health and social needs while promoting wellness and timely return to work.

This role requires travel up to 50-75% and the ability to work from home.

Qualificações

• Master’s degree in behavioral/mental health or related field.
• Licensed or eligible for licensure (LCSW/LCPC preferred).
• Strong communication, analytical and organizational abilities.
• Experience in case management, discharge planning or home health care coordination.
• Ability to work remotely with reliable transportation and travel 50–75%.

Responsabilidades

• Perform comprehensive assessments of members’ needs and eligibility.
• Coordinate care and facilitate access to benefits and services.
• Provide crisis intervention and refer to clinical providers as needed.
• Collaborate with Medical Directors and care teams to optimize outcomes.
• Document care plans, monitor progress and ensure regulatory compliance.
