Job summary

CVS Health is seeking a full-time Case Manager in Illinois to collaborate with members, providers, and community resources. You will assess needs, coordinate care, and advocate for services to ensure quality outcomes.

The role requires travel within the Chicago area and an emphasis on behavioral/mental health coordination. Qualified candidates hold a Master’s in Behavioral/Mental Health, an IL license (LCSW/LCPC), and 3–5 years of clinical experience.

Qualifications

• Must reside in Illinois.
• 3-5 years clinical practical experience.
• 2-3 years CM, discharge planning and/or home health care coordination experience.
• Willing and able to travel 50-75% in South Loop, Chinatown and surrounding areas.
• Certified Case Manager is preferred; bilingual is a plus.
• Master's Degree in Behavioral/Mental Health or related field.
• LCSW or LCPC licensed in Illinois.

Responsibilities

• Assess referred members’ needs/eligibility and determine approach to case resolution.
• Apply clinical judgment to strategies reducing risk and addressing complex indicators.
• Perform crisis intervention and provide crisis follow-up.
• Interpret criteria and guidelines to ensure appropriate administration of benefits.
• Collaborate with supervisors/Medical Directors to overcome barriers and achieve outcomes.
• Develop and monitor plans of care with the member and care team.
