The role emphasizes independent work, crisis and discharge planning, and collaboration with physicians, providers and community resources.

Qualifikationen

• Must reside in Illinois.
• 3-5 years clinical practical experience.
• 2-3 years CM, discharge planning and/or home health care coordination experience.
• Work from home with independent collaboration v ia virtual tools.
• Must have reliable transportation and travel up to 50-75% in designated areas.
• Excellent analytical and problem-solving skills.
• Effective communication, organizational and interpersonal skills.
• Ability to work independently and navigate multiple systems.
• Proficiency with MS Word, Excel, Outlook and PowerPoint.
• Preferred: Certified Case Manager; bilingual abilities.

• Assess referred members' needs/eligibility and plan care.
• Coordinate care using collaborative processes with care teams.
• Provide crisis intervention and follow-up as clinically indicated.
• Apply criteria and guidelines to ensure appropriate benefits and care.
