Job summary

CVS Health is seeking a full-time Case Manager in Illinois to assess members’ needs, coordinate care, and ensure appropriate benefits delivery. Qualifications

• Must reside in Illinois.
• 3-5 years clinical practical experience.
• 2-3 years CM, discharge planning and/or home health care coordination experience.
• Confidence working from home and collaborating virtually.
• Must have reliable transportation and be willing to travel 50-75% in specified areas.

Responsibilities

• Assess referred members’ needs/eligibility and determine approach to case resolution.
• Apply clinical judgment to reduce risk factors and address complex indicators.
• Provide crisis intervention and follow-up for members in crisis.
• Interpret criteria and guidelines to ensure appropriate administration of benefits.
• Collaborate with supervisors and Medical Directors to overcome barriers to goals.
