Qualifikationen

• Must reside in Illinois.
• 3-5 years clinical practical experience.
• 2-3 years CM, discharge planning and/or home health care coordination experience.
• Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually.
• Must possess reliable transportation and be willing to travel up to 50-75% in specified areas.
• Excellent analytical and problem-solving skills.
• Effective communications, organizational, and interpersonal skills.
• Ability to work independently.
• Proficiency with MS Word, Excel, Outlook and PowerPoint, plus proprietary applications.
• Efficient and effective computer skills including navigating multiple systems.

• Collaborate with Medical Directors and programs to improve outcomes.
• Provide crisis intervention and follow-up as needed.
• Monitor plans of care; ensure regulatory and accreditation compliance.
