This full-time role requires 3–5 years clinical experience, an MSW/LCPC/LCSW preferred, and ability to travel up to 75% across Southloop and nearby areas; remote work options and strong communication skills

Qualificações

• Master's degree in Behavioral/Mental Health or related field.
• LCSW or LCPC license in Illinois preferred.
• 3-5 years of clinical practice experience.
• 2-3 years of case management, discharge planning, or home health care coordination experience.
